As a Licensed Practical Nurse with our agency, we are seeking someone to join our team to help facilitate nursing functions that supports individuals with intellectual and developmental disabilities and their well-being. In this position, you will be working in a team environment within the Nursing Department. This position as the LPN will facilitate physical assessment functions, document assessments appropriate to licensure, care planning, and medication administration monitoring. The LPN will be responsible for providing employee training on medical support for clients and to employees on medical and/or health objectives specific to the needs of the client’s support. In this position, the medical environment will be conducted in an in-home care environment.
Essential Functions:
The Licensed Practical Nurse (LPN) will conduct physical assessments for assigned individuals, ensuring timely and comprehensive evaluations. This role includes providing support and training to staff to maintain safety and address medical concerns, fostering continuity of care for the individuals we support. The LPN will oversee the medication administration process to promote optimal health outcomes, which involves managing medication orders, consulting pharmacists on Pharmacy Consultation contracts, addressing and retraining staff on medication errors, reviewing Medication Administration Records (MARs) and count sheets monthly, supervising medication administration, and conducting random audits.
Additionally, the LPN is responsible for managing medical and health needs by responding promptly to concerns, performing tasks in line with medical procedures, scheduling and arranging transportation for appointments, assisting with hospitalizations, coordinating aftercare, and ensuring ongoing monitoring following discharge. The LPN will provide direct support and monitoring to individuals, engaging them in their medical care at their developmental level and ensuring clear communication. This role also involves overseeing access to medical equipment, ensuring individuals have appropriate access to their environment and necessary interventions. All communication will comply with confidentiality and HIPAA privacy policies, with other medical tasks assigned as needed to ensure the safety and well-being of those supported
Qualifications:
- Current and in good standing LPN Licensing in the State of North Dakota
- Current and valid driver's license and proof of auto insurance.
- Successful completion of criminal background, child abuse and neglect check(s), credit check, and pre-employment drug testing.
